Steps to Correct Ayushman Card Details
1. Visit the Nearest CSC or Empanelled Hospital
Go to your nearest Common Service Center (CSC) or PM-JAY empanelled hospital. Carry valid ID proof such as:
- Aadhaar Card
- Ration Card
- PM-JAY family ID (if issued)
- 2. Request for Beneficiary Identification
- Ask the CSC operator or Ayushman Mitra to search your record using:
- Aadhaar number
- Ration card number
- Mobile number
- If your record appears, but with errors, proceed with the correction request.
- 3. Submit Correction Request
- You will need to submit a request for correction with valid proof. For example:
- Submit Aadhaar card if your name or DOB is wrong
